Intake, Assessment & Referral Services

HHRC  provides  education, counseling and HIV testing Referrals  We also reffe and  escort at-risk individuals to Detox Units HHRC is able to conduct a preliminary assessment, prioritize needs, and create an individual treatment plan that addresses each individual’s  needs via referral, follow-up and supportive services. This Program is led by the Intake Coordinator who works closely with an Intake Assistant to assure that clients access a comprehensive range of primary care and supportive services consistent with their personal needs profile.



Programs
Family Food Voucher Program (FFVP)

The purpose of the Family Food Voucher Program (FFVP) is to assist  eligible  families residing on Staten Island during the years to be  able to have a basic meals.FFVP provides financial relief in the form Food  vouchers, which must be used at one of three local supermarkets. Vouchers are disbursed once a month for one year.  


Alcohol and Substance Abuse Counseling

Initial Evaluation - This is a complete evaluation that includes a full history,    It generally takes 1-2 hours.

At the end of this visit, Our CASAC  will create a treatment plan specifically for you.

Follow-Up Visits – The need for additional visits are addressed in the treatment plan.
